We are thrilled to be representing one of Sheffield’s most valued and well-established charities as they recruit an experienced Support Worker to join their fast-growing team based in Sheffield. We have positions available within the team for both full and part time workers due to continued growth within the charity. We are looking to speak to caring, compassionate and self-motivated individuals that have a brilliant attitude and wish to make a huge difference to older and vulnerable people within our community.
The successful candidate will play a vital role within the organisation, working closely with all departments across the organisation. This role will involve working closely with older people and therefore a flexible and adaptable approach will be essential. The charity that we are recruiting for have various schemes to support older people in the community. This role will span across all schemes and the successful candidate will be responsible for working closely with the wider team to ensure that all services are successfully delivered. The schemes within the charity include; friendly visiting to help combat loneliness, hospital to home to help transport patients safely from hospital back to their home, A&E to home which is aimed at supporting older people who have visited A&E and don’t require any further treatment and Back Home which involves a 4-week intervention after being discharged from hospital. Duties and responsibilities for this role will be very varied and will include the following; -Taking referrals from a range of referrers (including health and social care professionals, friends and family members) to carry out the referral or involve volunteers -Ensure that all referrals are completed -Ensure that hospital dispatches have been arranged in a timely manner -Getting involved with low-level interventions with service users to help with their recovery and reduce the need for them to be re admitted to hospital -Help service users that have been discharged from hospital to re-gain their confidence once they are back home -Social interaction with service users to ensure that they are supported -Handle any enquiries received from service users or their families -Ensuring service users that have more complex needs receive a visit each month -Providing telephone-based support to service users when required -Maintaining accurate records in line with the charities processes and procedures -Attending regular training sessions relating to this Support Worker role -Other duties as required in line with the role
We would love to hear from candidates that have the following skills and experience; -2 years + experience working in a similar role or similar organisation -Experience working in a community environment would be advantageous -Experience working with volunteers would be helpful - Good educational background including GCSEs in Maths and English -Qualifications related to Health and Social Care would be welcomed although not essential -Excellent understanding of the needs of older people -Exceptional communication skills – both written and verbal -Superior organisational skills -Experience keeping accurate notes -Confidence using IT including the full suite of MS Office -Highly motivated and able to work well alone -Car driver with full access to a vehicle is essential – mileage will be paid in line with company policy -Clear Enhanced DBS Please note that the charity is not CQC regulated and therefore there are no personal care responsibilities within this role.
